Any persons witnessing a violation should report the same to the region in writing immediately. 

• David Richards suspended from IVA/USAV from April2004 through October 31, 2015. Mr. Richards is suspended during this period of time from any involvement and/or membership in the USAV and the IVA.
• PLAYERS VBC is suspended from IVA/USAV from June 6, 2005 for lifetime.
• Penina White is suspended from IVA/USAV from May 23, 2007 through October 31, 2008. Ms. White is suspended during this period of time from any involvement and/or membership in the USAV and the IVA.
• Rose Mapu Tuaitanui is suspended from IVA/USAV from May 23, 2007 through October 31, 2008. Ms. Tuaitanu is suspended during this period of time from any involvement and/or membership in the USAV and the IVA.
• Meliame Tukuafu is suspended from IVA/USAV from May 23, 2007 through October 31, 2008. Ms. Tukuafu is suspended during this period of time from any involvement and/or membership in the USAV and the IVA.
• Rosie Afo is suspended from IVA/USAV from May 23, 2007 through October 31, 2008. Ms. Afo is suspended during this period of time from any involvement and/or membership in the USAV and the IVA.
• Royal VBC is suspended from IVA/USAV from May 23, 2007 through October 31, 2008.
